to prevent blur, you can either
1. bump up iso
2. faster shutter speeds ( since u're using M )
3. larger aperture ( maybe you can try using A instead? "semi-auto" may work better for you now )
4. shorter focal length (wide end of your kit lens)
5. use flash with diffuser ( like your closeups of the guitar )
6. train your handheld skills ( inhale deep, exhale then shoot )
7. forgot to turn your IS on ?
for OOF shots, select just 1 AF point and not auto select points. keep the body stable and shoot, shouldn't be a problem.
i'm a noob too, so that's what i can come up with for now. definitely not exhaustive.

Thanks for the reminder. Too bad, I don't have flash (except for the built-in one).
IS is on. prob the breathing and handheld thingy might need improvements.
