Just sign a model release and get ur mum to countersign and I'll be fine with it.
Bring a chaperone at least older than 18, will sign a affadavit to affirm that this shoot is valid.She'll also help you do your makeup.

The only time I know you need model release form is when you plan to make money out of the photos, or want to be able to do anything you want with the photos. That's when you need to bind the model legally such that you can use the model's images without paying the models anymore money. You need minors to get guardians/parents to sign so that they can't break the contract, that's all.

Of course there is no "perfect" model release form. The proper one would be a form that protects the model and the photog too.
I think you got your facts wrong on this, the point quoted by you is only relevant to the american legal system and is not related to crown law.(of which SG is using a derivative of)
Would love to see you go ahead and get a model shoot without a release and see her get an injunction against you or better yet... make a police report to get you, your assets....(That means your cameras,your pictures, your pc all seized in the name of an investigation) and if you're lucky a frontpage article on our favorite local daily.

Well, the judicial system is littered with hundreds(at least) of examples of well meaning photogs who do so and have paid the price. just look at the recent http://www.chattanoogan.com/articles/article_51870.asp (not exactly on topic but close)
All it takes is for a "concerned" parent(financially maybe?) to flip allegations your way like " I think he took advantage of my daughter/son" "Do you think that he coerced him/her into doing his photoshoot?" He/She is still legally a minor and that can land you into a nice big pot of hot soup.(Think no job, search and seizure)
So by all means, please don't get the girls to sign.(SIC!)
Btw, the affadavit is to certify that nothing untoward happened during the photoshoot and will provide you with a material witness whose testimony is unimpeachable. This may happen if he/she decides that you look like a bank and changes his/her statement.
