i am using the 5D2 + 70-200 Mk2 f2.8 with the new Canon 2X converter. IQ is ok and still able to crop further and AF is not an issue.

MK2 at f2.8 is way sharper than MK1

every camera behaves differently from another camera. so back focusing may not be a faulty lens. send for calibration.

Get anything that is from low to medium strength. Do not use the high strength loctite. Apply on the screw thread. There shouldnt be any residues if you do not over apply.

A 5DMK3 and a 85mm f1.8 may help.
