You might want to try to shoot at higher aperture (e.g. F4). So when you focus lock and re-compose the image, you will have higher chance of getting sharp image. As you skill improved, you might want to try F2.8 or lower.

In my opinion, exposure is fine with the use of fill flash. You might want to consider to research more on composition, model's pose and model's expression.
