Have a look at www.blurb.com they do decent books at good prices. I've seen a book produced by Apple so if you're a mac user it might be worth investigating that option, they're more expensive than Blurb but slightly better quality.

Not the best solution in the world, but I have seen people put some little spots of blu-tak on the end of the lens and then used that to hold the filter in place.
